Tax Information Check List
If you are a new client a copy of your previous year’s tax return.
- W-2 forms for wages received from employers for yourself, spouse, and other dependents, and W-2G statements for gambling winnings.
- 1099-DIV, 1099-INT, and 1099-OID forms for interest and dividends received by you, your spouse, and your dependents.
- 1099-MISC forms for miscellaneous income.
- 1099-R forms for pension and IRA distributions.
- 1099-B statements from brokers for sales of stocks, information on original purchase of stocks including date purchased, number of shares purchased, amount paid, and amount of dividends reinvested.
- 1099-G tax refunds & unemployment compensation.
- Other 1099 forms including 1099-A (acquisition/abandonment), 1099-C (discharge of debt), 1099-PATR (patrons of cooperatives), and 1099-LTC (long-term care), and any other form 1099 received.
- Form 1098 received from banks indicating mortgage interest paid, including home equity loans, and real estate taxes paid on all properties.
- Statements received from colleges including 1098-T, and summaries from personal records for amounts paid for tuition & fees for yourself, spouse, & other dependents.
- Interest paid on school loans including 1098-E, and the date you made your first payment.
- Information, including birth dates and social security numbers for ALL dependents.
- Payments to IRA, Simplified Employee Pension, and Keogh plans, including rollovers and year-end account statements.
- Medical and dental expenses, including health insurance, long term care payments, and vehicle miles.
- Contributions to charitable organizations (both cash and non-cash) including statements received & expenses related to volunteer work. If non-cash, the name and address of charitable organization.
- Miscellaneous deductions such as union dues, investment advisory fees, subscriptions, safety deposit boxes, etc.
- Unreimbursed employee business expenses such as expenses for transportation and lodging, business meals, entertainment, continuing education, tools, uniforms, etc.
- Business income and expenses, including information on purchases and sales of equipment.
- Rental income and expenses on all properties owned.
- K-1 forms for partnerships, estates, sub chapter S corporations.
- Vehicle expenses, including total miles for the year, total business miles, or actual expenses, including lease (car value) and loan information, if you use your automobile for business and employee related purposes.
- Closing papers from sales, purchases, and refinancing of properties, including Form 1099-S.
- Child care payments, including the name, address, and social security or ID number of the child care provider.
- Moving expenses, including transportation and lodging.
- Casualties and thefts, including insurance related statements.
